Annual Conference 2015 - Forms
The Conference brochure and the booking form can be downloaded using the links at the bottom of the page.
They are Adobe Acrobat documents and you will need the free Adobe Reader to access them.
Making your booking
Please do one of the following:
1. Complete the booking form document electronically on your computer by typing into the blue boxes. Then save it and e-mail it to bpfconference2015@gmail.com. You will still need to send your deposit/payment by post or via BACS.
2. Complete the form electronically as above, but then print out the completed form, sign it, and send it - with your deposit - to the address given.
3. Print out the booking form, fill it in by hand and post it, with your deposit, to the address shown.
In all cases, your booking will not be complete until your payment has been received.
